Who This Service Supports

Specialised Support Coordination is for NDIS participants facing complex challenges and significant barriers to accessing supports. This includes people at high risk of homelessness, individuals involved with the justice system, those with complex challenging behaviors, significant mental health or alcohol and other drug (AOD) issues, or participants with minimal or no informal support networks.

This service bridges the gap between standard support coordination and high-intensity clinical care, focusing on solving specific complex problems, stabilizing your support environment, and enabling you to work towards greater independence.

From there, we’ll work with you to either develop a support plan tailored to your unique circumstances, or connect you with services based on your current plan. We’ll discuss timelines, introduce you to potential support workers and ensure everything is in place before services begin.
